This accessible study about the collusion between medical science and the drug industry emphasizes how drug companies market their products by either redefining problems as diseases (like female sexual dysfunction) or redefining a condition to encompass a greater percentage of the population. Moynihan, a health journalist for the New England Journal of Medicine and the Lancet, and Cassels, a Canadian science writer, note, for instance, that eight of the nine specialists who wrote the 2004 federal guideline on high cholesterol, which substantially increased the number of people in that category, have multiple financial ties to drug manufacturers. Physicians now routinely prescribe cholesterol-lowering pills (statins) that may have perilous side effects, when many people could lower their risk of heart attack with less costly and dangerous steps, such as exercise and improved diet. Through aggressive merchandising, funding of medical conferences and expensive perks, drug companies win doctors over to diagnosing these "diseases" and prescribing drugs for them. Unfortunately for these authors, much of this territory has been covered by several books in the past year, most notably Marcia Angell's The Truth About the Drug Companies (due out in paperback from Random House in September). Since there are no pills to combat these chemicals, the authors have developed a five-week, self-directed recovery program based upon mind-body principles. The book is divided into three parts: "Understanding Fibromyalgia," "Tools To Battle Fibromyalgia," and "The Five-Week Plan To Battle Fibromyalgia." The structured program the authors have developed focuses on meditation, journal writing, and self-talk aimed at creating a psychological awareness of the emotions, thoughts, and feelings that produce pain in the body. At first glance, this book's photos and illustrations, which are just as important as the text in an exercise book, seem dated and bland. But a reading of the text and a closer examination of the illustrations reveal that what makes this yoga text different and worthwhile is the author's commitment to tying yoga's spiritual aspects to its physical components. Farhi (The Breathing Book) discusses the "Ten Living Principles," or the yamas ("wise characteristics") and the niyamas ("codes for living soulfully"), and also considers the importance of the asanas ("postures") in grounding spirituality in the body. However, Farhi does far more than provide descriptions and illustrations of postures. At the beginning of each new section, she has readers move through one or two core poses. She then asks them to focus on the way they feel physically while performing the pose. Using these core poses, she moves on to postures of greater complexity while allowing for different levels of ability. One possible problem for beginners is her use of posture names to describe poses that involve movement from one posture to the next (e.g., the sun salutation or the downward dog). Addiction expert DesMaisons, who addressed adult "sugar sensitivity" in Potatoes Not Prozac, now turns her attention to the younger generation. Sugar sensitivity can be linked to behavior, health and emotions, she says. In kids who are especially vulnerable to sugar, imbalances in brain and body chemistry are the cause of low serotonin, low beta-endorphin and volatile blood sugar. Consequently, kids react to sugar as if it were a drug, embarking on a craving and crashing cycle. DesMaisons advocates modifying children's biochemistry by changing their diets, and suggests parents explain to kids that sugar has a connection to their mood and feelings, and then get them to help keep a food journal. She also suggests parents alter their own use of sugar before they start on their child. Although DesMaisons advocates cutting out sugar, she isn't a purist; hot dogs and chips are occasionally allowed, along with healthy snacks and whole wheat bread. Though some parents may balk at the monumental nature of the task, DesMaison's sugar-free plan is practical and gradual, and she includes kid-tested recipes as well. DesMaisons (The Sugar Addict's Total Recovery Program) asserts that sugar not only affects behavior but for some people with a sugar sensitivity also becomes a drug. Addressing parents of children of different ages, she explains that sugar sensitivity is the root of many behavioral problems. DesMaisons began developing her theory as the director of an alcohol treatment center; it's not surprising, then, that this reads much like a manual for parents of a child in drug or alcohol treatment. DesMaisons provides excellent suggestions for involving children in the process, as well as excellent recipes. However, given her controversial topic, she should have directly referenced medical literature rather than throwing all her sources together in the bibliography. Also, her resource section might have been more useful had it included more than a glorified ad for her consulting firm and its products. Depression has become the single most commonly treated mental disorder, amid claims that one out of ten Americans suffer from this disorder every year and 25% succumb at some point in their lives. Warnings that depressive disorder is a leading cause of worldwide disability have been accompanied by a massive upsurge in the consumption of antidepressant medication, widespread screening for depression in clinics and schools, and a push to diagnose depression early, on the basis of just a few symptoms, in order to prevent more severe conditions from developing.
In The Loss of Sadness, Allan V. Horwitz and Jerome C. Wakefield argue that, while depressive disorder certainly exists and can be a devastating condition warranting medical attention, the apparent epidemic in fact reflects the way the psychiatric profession has understood and reclassified normal human sadness as largely an abnormal experience. With the 1980 publication of the landmark third edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-III), mental health professionals began diagnosing depression based on symptoms--such as depressed mood, loss of appetite, and fatigue--that lasted for at least two weeks. This system is fundamentally flawed, the authors maintain, because it fails to take into account the context in which the symptoms occur. They stress the importance of distinguishing between abnormal reactions due to internal dysfunction and normal sadness brought on by external circumstances. Under the current DSM classification system, however, this distinction is impossible to make, so the expected emotional distress caused by upsetting events-for example, the loss of a job or the end of arelationship- could lead to a mistaken diagnosis of depressive disorder. Indeed, it is this very mistake that lies at the root of the presumed epidemic of major depression in our midst.
In telling the story behind this phenomenon, the authors draw on the 2,500-year history of writing about depression, including studies in both the medical and social sciences, to demonstrate why the DSM's diagnosis is so flawed. They also explore why it has achieved almost unshakable currency despite its limitations. Features:In their discussion of the flaws in our definition of depression, the authors detail a number of reasons why normal, human sadness has been wrongly included as a depressive illness. They begin with a historical examination of depression versus sadness and proceed forward in time to consider the modern contributors to the misunderstanding of sadness. The authors consider a number of reasons for this, but a few stand out: the DSM and its definition of depression that needs more qualification to distinguish it from sadness more readily; scientific publication on depression that has become an industry unto itself; and the widespread availability of antidepressant medications.
Assessment:This is an interesting and thought provoking book that underscores the need to examine more fully each patient's psychological illness and the factors contributing to it. Though a well trained mental health practitioner should be able to distinguish sadness from depression, the authors do an excellent job of breaking down the flaws in the mental health field, particularly the problems with the DSM, that have led to the sudden explosion of depressive diagnoses over the past 25 years. I would recommend thisbook to anyone interested in understanding depression more fully and the place normal sadness has in our society.

It does, however, take some effort to pin down the source of Shomon's medical information. While she quotes from personal interviews with medical experts, she sometimes neglects to give full citations. Libraries that already own similar titles-e.g., Elaine A. Moore's Graves' Disease: A Practical Guide and Kenneth Ain and M. Sara Rosenthal's The Complete Thyroid Book-can safely pass on Shomon's book. The author, a physician trained in Iran (Your Body's Many Cries for Water), not only believes that drinking water is healthy, but makes extravagant claims for its curative powers. He recounts here how he first discovered the healing abilities of water, when he was a political prisoner more than 20 years ago. While incarcerated, he successfully treated with plain water the stress-induced peptic ulcers that afflicted some of his fellow prisoners. According to Batmanghelidj, most people rely on dry mouth to signal thirst and as a result are seriously dehydrated. He posits that symptoms like fatigue, anxiety or depression indicate dehydration, which may result in serious conditions such as asthma, hypertension, brain damage, cancer, constipation, allergies and obesity. In order to compensate for fluid lost through urination, respiration, and perspiration, Batmanghelidj suggests drinking a minimum of two quarts of plain water daily (not alcohol, caffeine beverages or juices). In order to maintain good health, he also advises ingesting half a teaspoon of salt to balance water intake. In addition, a diet consisting of 80% fruits and vegetables and 20% protein should be followed along with an exercise program. To back up his theories, the author presents a wealth of somewhat confusing medical data as well as patient anecdotes that testify to water as a cure for various diseases. Batmanghelidj does state, however, that those diagnosed with specific conditions should not stop taking their medications abruptly, nor should they embark on a water regime until they have consulted with their physician. The recipes blend nutrition into a meal and harmony into mealtime. Seinfeld writes that mealtime should be about "happiness and conversation" not about "power struggles and strife," and her plan diminishes nagging.
